3.12.3 asks for ongoing monitoring. Assessors ask for proof. If your monitoring produces alerts nobody records, it fails both.

Monitoring done right emits artifacts: what was watched, what changed, what was decided, and who decided it.

When the assessment comes, the monitoring itself is exhibit A.
